Errors occur with both paper systmes and EMRs. Replacing one with the other just creates a different type of error. EMRs are not the be all and end all, they are a tool. EMR vendors need to be held responsible for these errors–and that is where the fight is, as the HIT industry doesn't want to be held accountable. These errors need to be fixed, and transparency is a key to the fix.
